Please be advised that all clients must fill out a one-time Client Health History form before therapy can be performed. Just click on First-time Client Health History form below to get started. This form is intended to gather health information for safe, effective massage. It also serves as your consent for massage therapy services. Consent for the treatment of minors (under age 17) must be given by the legal parent or guardian; the legal guardian or parent must accompany the minor for the entire duration of the therapy session. 

Certain conditions may require some clients to obtain permission from a licensed medical professional prior to treatment. You may do so by clicking on and printing the Physician's Permission form below. You may also elect to use the Physician's Referral form below should a licensed medical professsional wish to prescribe massage therapy as part of a treatment plan.

I will use the Body Map for Clients as an ongoing tool and record for monitoring and measuring muscular function, deviations and improvements. Notes will be made following each session.
